Program Admission Criteria

MTH provides a transition/case management worker for each applicant.  The applicant must be highly motivated to engage in goal setting, and developing and implementing an individualized program plan to improve housing stability and overall quality of life issues.

Age:  Minimum 18 years of age for admission to the program.

Screening:  The applicant will engage in a vulnerable sector police record check and disclose any pending charges.

Financial Status: The applicant must have a source of low income. The applicant will be fully responsible for telephone, internet and cable.  Financial disclosure and housing reference checks will be necessary.

This unit is a Region Rent Subsidy apartment whereby the heat, hot water, water and hydro may be included in the rent.  A working refrigerator and a working oven and stove are included in the rent.

Individual circumstances may be reviewed on a case-by-case basis. Participants meeting criteria will meet with the Client Services Committee for review, pending admission to the program.

A formal application requires the download, completion and return of this form. A decision will be based upon this more detailed application and subsequent interview. It can be completed by a community partner or by an applicant themselves.
